EpicCare Link Access – New Account Requests
OakLeaf Clinically Integrated Network offers EpicCare Link to qualifying organizations to access a patient’s OakLeaf’s Clinically Integrated Network medical record information for continuity of care and billing.
Qualifying organizations are organizations that share a common patient with one of the Oakleaf Clinically Integrated Network clinics on Epic Garden Plot and have been approved by the Network to receive access. Current clinics on Epic Garden Plot include OakLeaf Clinics, Eau Claire GI, Evergreen Surgical, Western WI Urology, and Foot and Ankle.
Step 1: EpicCare Link Site Level Agreement & Questionnaire
Organizations are required to have a completed EpicCare Link Access Agreement and Questionnaire.
- Using the links above, download and complete the EpicCare Link Access Agreement and Questionnaire.
- Agreements must be signed by a person having signature authority for your organization.
- The “Participant” blank should be completed with your organization’s legal name.

Step 2: New Account Request: EpicCare Link Access
- Click this link to request a new account.
- Choose “Oakleaf Medical Network” as the organization. Once records are generated, the users will receive an email to complete the account setup.
