MyChart is an easy-to-use, secure patient portal that lets you access your health information online.

MyChart allows you to access your medical record, send a message to your provider, request an appointment, manage your communications, obtain proxy access to your family’s records, update your insurance, and make a payment all from the convenience of your computer or phone.

You will need to merge your accounts.

If you’ve used MyChart in the past, you will need to merge accounts. Search “Link” in the menu. Select HSHS and complete the verification. Now, you will be able to see your health history from the HSHS MyChart on the new OakLeaf Clinics MyChart!

Set your preferences on which communications you receive by email, text, and mail as well as which ones are pushed to your phone.

Linking Your Accounts

Gaining Proxy Access
Are you the medical decision-maker in your family? Do you care for a parent or elderly relative? With proper consent, you may use the features of MyChart to manage the health information of your spouse, children, and elderly relatives.

Children Up to Age 12

Children 13-17
Due to Health Insurance Portability and Accountability Act (HIPAA) regulations, we cannot offer full proxy MyChart access for children ages 12 to 17. Limited proxy access allows you to schedule appointments, change your address or other account information, request medical advice, or enter your child’s information.
